Oral Communication Waiver letter - COSC

Can someone provide me a standard letter if there is any for the waiver and advise on what needs to be submitted along with it to get a waiver for oral communication? I have extensive experience speaking to an audience of 30 people to 350 ppl as a part of my job and can send supporting presentations or if needed can make short video of it. Thanks.

The waiver request packet should include:
Completed and signed Application for Waiver Form (Required)
Letter to Provost requesting waiver (Required)
Supporting documentation demonstrating proficiency (Required)
Signed letter from Supervisor or other appropriate reference to corroborate expertise (Required)
