My evaluation was done in a similar way. (They may have changed how they do them. Mine was completed about 1 year ago.)
The top part (above the major) was what I would need to complete a BS in Liberal Studies without a major. Then the bottom part was what I would need if I chose to complete a major. So as you complete what you need for the major, you should also be able to cross off UL and LL credits needed in the top section. Also, if you take Stats for the major, that meets the Quantitative Requirement. If you complete the major, that meets one of the depths.
There should be exams to meet some of the major requirements.
Arts & Sciences means Humanities, Social Science, History, Natural Science, or Math.
