I have come across 3-4 occasions which I have flagged a question during a test and have never noticed a change to my scores or even a response. These were situations where SL was clearly incorrect with their chosen answer. No support ticket is created either. Curious if anyone has actually received any form of correction or response by flagging a question?

When I took courses with SL, the test bank system had quite a few errors.
What I would do is type up the question/answer. Made note of the correct answer (including book pg number) and shot it over to my rep. He would then review answers and adjust quiz/exam grade when appropriate. It did not occur often enough to affect my grade, but I hoped reporting the errors would assist their database cleanup.

There are a lot of typos in the quick exams. And some fill-in-the-blank questions require such exact wording that they should be removed. One was a "what is" question. I put Table of Elements. I got it wrong. They wanted Periodic Table. I think the actual title is Periodic Table of Elements. Someone could put The Periodic Table of the Elements, and they would still be wrong. Just a quick quiz, so not a big deal, but silly things like that should be changed or corrected.

Most tests that I've taken (in life) that allow you to flag a question is just a flag for you to review it after pressing the next button (you're not clear on the answer but want to circle back to it after you finish the rest of the questions so you don't run out of time).

Yes, the flag is just for you. It is not anything that has any bearing on your score or reports anything. It is just a marker. I use them often. For example, if I know that question 1 is going to require me to look up all 4 answers in the book and I am concerned about time I will skip and flag it so that I can be sure to answer all of the questions I do know. Or sometimes I'll choose my best guess and flag it then if I have time at the end I can go back and make sure

Keep in mind that SL does not create this content, they are simply a platform/provider. I'd be suprised if it led to a correction - you might, however, be able to score an extra point or two.

Agree with cookerosa. With that being said, I bet if you failed a course by a few points and petitioned a review with Straighterline, at worst case, they probably would let you redo the exam and at best case might secure those extra points (if agreed there was an actual error in the question/answer).
Between all point opportunities with exams, midterm & final, I just can't see a scenario that 3-4 exam questions would matter in passing a course. However, if it does happen, I would encourage you to give a request to Straighterline. Per my experience, they are always open to doing what's best for the student
