Failing Grade after Passing Final

Did anyone receive 4s on the final assessment, then receive a failing grade when the term ended?
All of the milestones were completed and affirmed. All other grades received. 

I emailed the professor, but haven't received an explanation.
Anyone else experience this?

There has been a glitch in the past and students have contacted their professor for it to be corrected.
