You can find a list of the exams (and their cost) here:

You'll need to look on the left side of the screen and click the area your exam falls under (Business, Sciences, etc) and then the exam name itself. You'll see the cost of the exam as well as a description when you do.
As for where to take them -
The last time I checked, it was still given by Pearson Vue. Perhaps one of the others here can confirm that. Since our local Vue site isn't a "Professional" site, I had to take mine at the testing center here on base with the old paper and pencil variety. My process for doing that was:
1. Talk to advisor and get approval to take the exam.
2. Sign up online at the above link, and pay for them (ouch)
3. Print off the approval and the order form and bring them to the base education office.
4. Take the exam
5. Mail it off for grading
If you have a "Professional" testing center nearby, then I'd imagine it's a much quicker process You'll still probably have to sign up and pay for them through excelsior, but from there I'd guess Pearson Vue takes over.
I know we have a few ECE takers here, so I'd hold off for their input if you plan on going the latter route.
