I've been doing a lot of research on this and many of the threads that came up were older ones.
I need to complete a college algebra class online by this summer, with an actual letter grade, in order to apply for a health occupation program locally. That rules out Straighterline because I'm not going to one of their partner colleges and I need an actual letter grade.
I am looking for one less than $600. I took and passed intermediate algebra. But math is not my strong point. I am fine with proctored exams, but I am looking for a course that allows calculators and potentially open book exams or use of notes.
Any advice?

If you're eligible for any financial aid to offset, AMU/APUS is $250/credit ($750 for the class). It allows you to use calculators and your notes. If this won't work, I hope someone else has a better solution.

You might also consider your local community college. Many offer online classes now. Of course it depends where you live, but they are usually cheaper than other sources. You should be able to register as a non-matriculated student and take whatever courses you need without having to deal with a placement exam (confirm with admissions).
I noticed when taking OL courses at my CC both the midterm and final were delivered via the class dropbox about 20-30min prior to class start. You had the full class to work through the exam, fill in the answers on the word doc, then upload to your assignments submission page.
The major benefit of taking the course locally is, if you get stuck in certain areas you can always visit the math lab for free tutoring sessions.

Check the independent study offered by university of idaho and U of Arkansas. Likely the cheapest. Also consider just taking it as a regular course through New Mexico junior college e campus

BYU Independent is both self-paced and rigorous. This is great option for $519

I second the community college option. They probably have butt in seat as well as online. MANY CC's offer 2 of the 8 week terms in 1 16 week semester, meaning it's not too late. You're probably going to get tuition at the best rate (~$100/credit), new books are always expensive, search online to find a used version for less. If you have to buy an online math code, there are not many options around that (worth posting on local craigs list). Good luck!

Definitely check into your local community college. I took College Algebra online at my local CC. I preferred this option because it was cheap (~$450 for the tuition and Math software all covered by Financial Aid) but I could also study around my busy schedule and visit the campus tutoring as needed.

New Mexico Junior College = $62 per credit + $17 usage fee = $79 per credit x 3 = $237 + $40 internet fee per class = $277 total for the course. I could not find anything better in my state or online.
This is the syllabus: NMJC Course Syllabus It does not say whether or not the tests are open book, but I'm sure you can find out by contacting the instructor. Graphing calculators are required, so I wouldn't see them being forbidden to use with tests.
