Alissa has a great blank template you can use for the BSLS... You can also modify it slighty for use for a Major or Area of Focus.

As far as what would count as upper levels, in theory, anything that you have that starts with the class number 3 or higher should be upper level, such as
ECO - 36100 Essentials Of Mktg 3.0

You'll want to check your school about the classes that are listed as course 00000 such as
GE1B - 00000 Linear Math Analysis 3.0
