If for example I took English 101 at school1 for a D and later for an A at school 2, will they only count the A or both grades?

Depends on what type of program you're going for. MD programs will calculate all GPA no matter what, DO programs will take into account the higher of the 2 grade retakes.
For average to low tier graduate programs, they may/possibly look at the last 60 credits you've taken, and may or may not look at the higher of 2 grades. For a lot of lower tier grad programs, they may typically have no GRE/GMAT requirement, and relatively low GPA requirements (like a 2.75 or 3.00 or something along those lines).
For high tier/competitive graduate programs, they will likely calculate both grades.
Of course, these are generalizations, but that's how it normally works.
